This information collection request (ICR) form will be used to more efficiently collect the information required by FMCSA to process name and addresses and reinstatements of operating authority requests. For-hire motor carriers, brokers and freight forwarders are required to notify FMCSA when they change the name or address of their company. This multi-purpose form will simplify the process of gathering the information needed to process the entities' requests in a timely manner, with the least amount of effort for all parties involved.

This program change increase of 5,575 estimated annual burden hours is due to a new ICR that will be used to more efficiently collect information the FMCSA requires to process name and address changes and reinstatements of operating authority requests.
