Abstract: This information collection request (ICR) form will be used to more efficiently collect the information required by FMCSA to process name and addresses and reinstatements of operating authority requests. For-hire motor carriers, brokers and freight forwarders are required to notify FMCSA when they change the name or address of their company. This multi-purpose form will simplify the process of gathering the information needed to process the entities' requests in a timely manner, with the least amount of effort for all parties involved.
