Get into your Sanctuary Day Customer Satisfaction Survey

• Your participation in this survey is voluntary. There are no penalties for not answering some or all of the questions. This study is being conducted by ****FILL IN SANCTUARY NAME**** and the National Oceanic and Atmospheric Administration. This information will be used to improve visitor satisfaction with National Get into your Sanctuary Day-associated activities. At the end of the evaluation, any materials identifying you as an individual will be destroyed.
