Abstract: The three forms serve the functions of the Universal Service Schools and Libraries Support Mechanism, 47 U.S.C. Sec. 254 et seq. They are used at the point were services provided pursuant to the program are implemented, or about to be implemented, and are a necessary prerequisite to the distribution of payments under the program. The forms ensure that congressional mandates regarding safer access to the Internet by minors and adults are met.

Short Statement: The Commission is reporting program changes/decreases to this information collection. There are program changes/decrease to the number of respondents by -12,500, total annual responses by -12,500 and total annual burden hours by -35,200 which are due to the impact of the removal of the technology plan certification on the FCC Form 486 for funding year 2016 and later and the new availability of the FCC Forms 486 and 500 through an online platform. Also, the Commission is reporting adjustments/decreases to this information collection. These adjustments/decreases are to the number of respondents by -14,500, total annual responses by -14,500 and total annual burden hours by -10,875; these were overstated in the Commission’s previous submissions to OMB.
