Abstract: This information collection is submitted on occasion by state member banks (SMBs), bank holding companies (BHCs), savings and loan holding companies (SLHCs), and foreign dealer banks that are municipal securities dealers. The Form MSD-4 collects information (such as personal history and professional qualifications) on an employee whom the bank wishes to assume the duties of municipal securities principal or representative.
