According to the Paperwork Reduction Act of 1995, an agency may not conduct or sponsor, and a person is not required to respond to, a collection of information unless it displays a valid OMB control number. The valid OMB number is 0503-0261. The time required to complete this information collection is estimated to average 10 minutes per response, including the time for reviewing instructions, searching existing data sources, gathering and maintaining the data needed, and completing and reviewing the collection of information. Response to this survey is voluntary.
The Illinois Department of Agriculture (IDOA), Bureau of Environmental Programs is charged with the administration of various programs and services including: the testing and licensing of pesticide applicators and operators; the registration of pesticide products for sale in the state; the investigation of alleged pesticide misuse; annual inspection of the state’s nurseries; the issuance of phytosanitary certificates to facilitate international trade of agricultural commodities; the detection, control and possible eradication of various invasive insect pests and plant diseases such as the gypsy moth, Asian Longhorned Beetle, and Emerald Ash Borer; the permitting and annual inspection of agrichemical and lawncare containment facilities and structures; and the review and approval of new and modified livestock manure storage and handling facilities. The IDOA Bureau of Environmental Programs is committed to delivering exceptional customer service to the people of Illinois.
