Title of Information Collection

WaterAlert SMS User Survey
Purpose

The WaterAlert SMS User Survey is intended to determine basic satisfaction for users who have opted in to receive
a customized notification of water activity through a service called WaterAlert. This survey informs us about overall
user satisfaction and will gather some additional characterization of what use case the sample of users are fulfilling
(such as to help support an emergency decision, or to launch a scientific action). This survey will not be collecting
PII; it is collecting information to see trends in our users without requiring specific PII. The results of this survey will
be used in determining user satisfaction and user demographics. Here is our proposed statement:
Paperwork Reduction Act Statement: We are collecting this information subject to the Paperwork Reduction Act (44
U.S.C. 3501) to enable stakeholders the opportunity to give feedback about WaterAlert. Your response is voluntary,
and we will not share the results publicly. We may not conduct, or sponsor and you are not required to respond to a
collection of information unless it displays a currently valid OMB Control Number. OMB has reviewed and approved
this survey and assigned OMB Control Number 1090-0011, which expires 10-31-2021 Estimated Burden Statement:
We estimate the survey will take you 2 minutes to complete, including time to read instructions, gather information,
and complete and submit the survey. You may submit comments on any aspect of this information collection to the
Information Collection Clearance Officer, US Geological Survey, [email protected].
Description of Respondents

The respondents have self-selected to receive a custom text message (SMS messages) from
USGS. They have provided their contact information through that service, and this survey will be
linked out in a text message.

We have a self-selected group of users who have signed up to receive a custom SMS notification
when a water event is occurring. We would send an SMS message to this group of users including
the link to the survey asking for their participation. Only the users who are interested in providing
feedback will click on the survey and have the burden of completing the short survey.
