Title of Information Collection

Pre-testing of Youth Quota Hunt Forms for Use by National Wildlife Refuge System
Purpose

The National Wildlife Refuge System offers hunting opportunities to youth hunters. The Service
developed two new forms to be pre-tested specifically with youth deer and turkey hunt opportunities.
The original quota hunt permit application (FWS Form 3-2354) is approved under OMB Control No.
1018-0040. The Service is actively working to revise all forms approved under 1018-0040 to make it
easier for refuges to customize forms for their specific needs, to include the ability to remove fields
that are not needed. Pretesting the two youth hunt forms would provide the refuges with a more
streamlined custom version of the form to be used during their Spring deer and turkey hunt
opportunities.
