Feedback on Decommissioned USGS Applications
As user of a USGS Water web application that has been, or is scheduled to be, decommissioned we
are asking for your feedback on how you may be impacted by this change. The feedback that you
provide will help us better understand how USGS Water web applications are used so that we may
make informed decisions about resources and services we provide to the public in the future. Your
participation is voluntary and your responses will be anonymous. We estimate the survey will take
5 minutes to complete. Thank you for providing us with valuable information about how you use
our science.

Paperwork Reduction Act Statement: We are collecting this information subject to the Paperwork
Reduction Act (44 U.S.C. 3501) to inform future water web application decisions. Your response is
voluntary and we will not share the results publicly. We may not conduct or sponsor and you are
not required to respond to a collection of information unless it displays a currently valid OMB
Control Number. OMB has reviewed and approved this survey and assigned OMB Control Number
1090-0011, which expires 10/31/2021.
