ODEP is requesting approval for a non-substantive change relating to the Authorization of Disclosure form that is part of approved information collection 201911-1230-001, in which RETAIN grantees administer an Authorization of Disclosure Form to prospective enrollees. One grantee has requested a Spanish-language translation of this document, for the purpose of administering this translation to prospective enrollees who can more easily understand the translation than the original document. Since each prospective enrollee will only complete one version of the Authorization of Disclosure Form, there will be no change to the burden in terms of hours or cost.
