SUPPORTING JUSTIFICATION
OMB Control No. 2130-0500
On July 30, 2021, the Office of Management and Budget (OMB) approved the
“Accident/Incident Reporting and Recordkeeping” information collection request (ICR) with
terms. One of the terms was for FRA to “update the paper form with a PDF Fillable version
within 6 months and submit a change request to this ICR upon completion.”
This non-substantive change updates the following FRA Forms: 1) by converting the forms from
paper form to PDF fillable version and 2) adding the new expiration date of July 31, 2023 based
on the last OMB approval of this information collection.
