This information collection requires the owner or operator of a vessel, within 5 days, to report a marine casualty. Additionally, the owner or operator of a vessel must report chemical testing of commercial vessel personnel. |

When must information be submitted to the Coast Guard? |
For reporting a marine casualty, within 5 days of the casualty. For chemical testing of commercial vessel personnel, according to the requirements in 46 CFR Part 16. |

All the above information may be submitted in writing or electronically via facsimile or e-mail. |
What happens when complete information is received? |
This information will be reviewed by CG investigators. No specific documentation will be issued by the CG. |

The Coast Guard estimates that the average burden per response for this report varies per information collection--about 1 hour for a report of marine casualty; and from about 4 minutes to 2 hours for chemical testing reporting and recordkeeping.
