Who must comply?

Owners and operators of commercial vessels.

What is this collection about?

This information collection requires the owner or operator of a vessel, within 5 days, to report a marine casualty. Additionally, the owner or operator of a vessel must report chemical testing of commercial vessel personnel.

Title 46 CFR Parts 4 and 16 are available at—https://www.eCFR.gov. For 46 CFR, select TITLE 46 – SHIPPING, and follow it to Parts 4 and 16.

For reporting marine casualties, see 46 CFR Part 4.

For chemical testing, see 46 CFR Part 16.

When must information be submitted to the Coast Guard?

For reporting a marine casualty, within 5 days of the casualty. For chemical testing of commercial vessel personnel, according to the requirements in 46 CFR Part 16.

How is the information submitted?

  • For reporting a marine casualty, the information is submitted on form CG-2692 (and CG-2692A, CG-2692B, CG-2692C and CG-2692D, if necessary) within 5 days of the casualty. Forms are available at https://homeport.uscg.mil Missions > Investigations.

  • For chemical testing of commercial vessel personnel, according to the requirements in 46 CFR Part 16.

All the above information may be submitted in writing or electronically via facsimile or e-mail.

What happens when complete information is received?

This information will be reviewed by CG investigators. No specific documentation will be issued by the CG.

The Coast Guard estimates that the average burden per response for this report varies per information collection--about 1 hour for a report of marine casualty; and from about 4 minutes to 2 hours for chemical testing reporting and recordkeeping.
