streamline the general informal complaint process, excluding
slamming and programming complaints. Form 475 allows the FCC to
collect detailed information from consumers on the practices of
telecommunications companies to assist in resolving informal
complaints and to collect data as a part of the investigative work
performed by federal and state law enforcement agencies. Form 475B

and/or indecent material that will be used to assist in the
