collection
FCC Form 2000 A through F, General
Complaint Forms, will allow the FCC to collect detailed information
from consumers on the practices of telecommunications companies to
assist in resolving informal complaints and to collect data as a
part of the investigative work performed by federal and state law
enforcement agencies. Form 475-B, Obscenity or Indecency Complaint
Form, allows the FCC to collect detailed data from consumers on the
practices of programming providers that may air obscene, profane,
and/or indecent material that will be used to assist in the
resolution of informal complaints and to assess the practices of
programmers. FCC Form 501, Slamming Complaint Form, is designed to
assist consumers in filing slamming complaints with the Commission.
The form is devised to ensure complete and efficient submission of
slamming complaints. The form will be available to consumers
electronically and in hard copy. The information will be used by
the Commission to provide redress to consumers and to act against
companies engaged in this illegal practice as soon as possible. FCC
Form 1088 A through H, Junk Fax/Telemarketing Form, is designed
specifically for complaints that involve (1) junk faxes, (2)
telemarketing (including do-not-call violations), and (3) other
related issues such as prerecorded messages, automatic telephone
dialing systems, and unsolicited commercial email messages to
wireless telecommunications devices (cell phones, pagers). The
Commission is submitting this non-substantial change request
submission to the Office of Management and Budget (OMB) to simply
add to this collection (3060-0874) a portion of the burden hours
and costs that were approved under OMB control number 3060-1148.
This modification is due to the filing of complaints alleging
violations of the video description rules now being able to be
filed via FCC Form 2000C. Therefore, pursuant to the video
description rules that will go into effect on July 1, 2012, a
portion of these complaints can now be filed on FCC Form 2000C,
which is currently housed under OMB collection 3060-0874. The
Commission received OMB approval for the filing requirement on
September 8, 2011. Also, we are asking OMB to approve the option to
attach files by providing a dropdown menu on all FCC Forms 2000A
through F. The updated draft FCC Form 2000C has been included in
the collection for public viewing and OMB approval.

Because of the new Video
Description rules that will go into effect on July 1, 2012, the
Commission made various non-substantial changes to FCC Form 2000C.
Those non-substantial changes extracted burdens associated with
filing consumer complaints regarding video description from OMB
collection 3060-1148 to 3060-0874. Therefore, the burdens for this
information collection have adjusted as follows: The number of
respondents has increased by +13, from 518,193 to 518,206
respondent, the number of annual responses has increased by +13,
from 518,193 to 518,206 responses, and the annual burden hours has
increased by +6, from 195,704 to 195,710 hours. The annual cost
remains unchanged. There are no program changes.
