THE CIRP FRESHMAN SURVEY INFORMATION SHEET
PURPOSE OF THE STUDY
You are asked to complete this survey as part of the Enhance Diversity Study, a national study on student experiences and
career choices. Your school is part of this study, conducted by the Diversity Program Consortium (Co-Principal Investigator:
Keith Norris, MD, PhD, Teresa Seeman, PhD, and Steven P. Wallace, PhD) in partnership with the Higher Education
Research Institute (HERI) at the University of California, Los Angeles. This study is designed to determine what happens to
students when they attend college. Since 1966, over 15 million students at over 1,900 colleges and universities have
participated in this research. The data gathered are used in studies designed to better understand student learning and
development and to help improve the quality of college education.
PROCEDURES
To participate in this study, please complete and submit the attached survey. Most respondents complete this questionnaire
in about 25 minutes, although individual progress will vary by how quickly you move through the questions.
By taking this survey, you are giving permission for your school to share information from your student record such as
demographics, the courses you take and the grades you receive in your courses with the UCLA team (for information on
item details, visit http://stembuild.umbc.edu/DPC_Survey_Details/). You are also giving the UCLA team permission to
share the information you provide on the survey with your institution for their review of local programs and to allow UCLA
to contact you every year or so for follow-up surveys to understand your career choices after you leave your school. You
may decide not to complete the survey for any reason at any time without consequence of any kind.
POTENTIAL BENEFITS TO SUBJECTS AND/OR SOCIETY
You will receive a $5 Amazon card by email if you complete the survey. You may have the opportunity to reflect on your
prior academic experiences and your expectations for college as you complete the survey, which may enhance selfunderstanding. Your responses to the survey also will be directly beneficial to your college or university, and may benefit

Please note that your responses will be used for research purposes only and will be strictly confidential. Any information that is
obtained in connection with this study and that can be identified with you will remain confidential. Your identified responses will
be returned to your institution for institutional assessment purposes. Your responses will also be shared with members of the
Diversity Program Consortium to understand how well students are prepared for and succeeding in biomedical research careers.
Information about you is protected by a federal Certificate of Confidentiality. This means that we cannot be forced to release
information about you for any legal proceeding, even if a court of law asks.
The Certificate allows us to use information about you for purposes of this research, or to disclose it for other research when allowed
by law. The Certificate requires other researchers to also protect information we share with them.
There are limits to this protection. The Certificate does not protect your information when:
